You can scan actual pages, and reduce it with editing software to the smaller size needed. Arrange it to print out so you can fold in half, and have 4 pages.
Stack as many of above as needed, staple the stack through the fold like a magazine.
For a cover, use stiff, thin material, like the backing from a pack of checks. Cover it with another reduced scan. Glue the outer most of the page stack inside the front and back cover.
Here is an awesome website that has tons of free printables for dolls and dollhouses. For a 12″ doll you would want “playscale”. It is the largest of the miniatures. Here is a link to a list of items you can print out in playscale. Scroll down a bit for a selection of books:
If you wanted to personalize it with a page of your own I would just create it in a program like Word and size it to fit the minis you print out.

There are various ways to make “mini-books” from polymer clay… the pages can be paper, or baked polymer clay or liquid polymer clay, or anything else you want. The covers can also be left plain, or made patterned or embellished, etc.

take one piece of card stock and cut it about 1 and 1/2 inches high and 2 and 1/2 inches wide, then cut regular paper, just a hair smaller, staple the paper in the middle of the paper and fold the whole thing to form a book, then open it, and glue to her hands or stitch it, they also sell tiny books at some book stores like borders, they are about 5 dollars, but really cute. i got one on tarrot to be held by a gypsy doll.
